I hereby acknowledge this release from liability for accidental injury or illness which may incur as a result of participating in any physical activity. I hereby assume all risks connected therewith and consent for my child to participate. I agree to disclose any physical limitations, disabilities, ailments, or impairments which may affect my child's ability to participate.

Please be advised that if there is a field trip/school activity you do not wish for your child to attend we will require written notification prior to the date of the event as this is considered consent for the entire school year
